  • Key values arrive late, out of order, or in incompatible formats.

What “clean, connected data” actually means

If you ask most leaders whether their organization has “good data,” they’ll say yes. But in healthcare, “good data” has a very specific definition, and most organizations aren’t close.

Clean data
Data must be normalized to standards like LOINC, SNOMED, and RxNorm. It must be structured, complete, and validated. If the data feeding your AI is inconsistent, ambiguous, or unstructured (hello, PDFs), the model will replicate those flaws.

Connected data
Data cannot live in islands. AI needs clinical, lab, device, claims, and encounter context to interpret what’s happening. If these systems don’t talk to each other, AI is effectively operating blind.

Contextual data
Context is the difference between insight and noise. Imagine:

  • A diagnosis code without encounter reason.
  • A lab value without the timestamp.
  • A medication list without adherence history.

AI cannot infer what the data doesn’t give it.

But when the data is clean, connected, and contextual, AI becomes both powerful and predictable.

The architecture that makes AI possible

This is the second truth ViVE attendees need to hear: AI doesn’t run on dashboards. It runs on architecture.

The models you see at ViVE assume a real-time, structured, interoperable data foundation underneath them, but most organizations still operate on delayed feeds, HL7 v2 messages held together with duct tape, and brittle point-to-point interfaces.

AI requires:

Real-time data movement: Models depend on live signals, not yesterday’s batch files. A late lab value is a meaningless lab value.

FHIR as the semantic backbone: FHIR isn’t a “nice-to-have” anymore. It’s the language that allows AI to understand and act on clinical events consistently.

Governed, compliant pipelines: AI outputs must be traceable. Input data must be auditable. Pipelines must enforce HIPAA/HITRUST rigor while supporting evolving models.

Flexible, composable system design: AI shouldn’t break every time a workflow changes or a partner updates their interface. Modern architecture makes AI pluggable, testable, and sustainable.

This isn’t the flashy part of AI, but it’s the part that makes AI real.

What happens when data quality is poor? Three stories from the field

The clearest way to understand the problem is to look at where things go wrong. Here are three examples pulled directly from real-world scenarios and healthcare data integrations that our team encounters.

1. The sepsis model that succeeded in pilot and failed in production

In the test environment, the model predicted deterioration accurately. In production, timestamps arrived late, vitals weren’t standardized, and signals were occasionally missing.

The model didn’t get worse. The data did.

2. The chatbot that gave the wrong advice

The logic was sound. But the system wasn’t passed encounter context (inpatient vs. outpatient), and it made several incorrect suggestions.

Again, not a model failure. A data routing failure.

3. The lab alerts that never fired

Labs lacked complete LOINC mappings. The alerting algorithm didn’t know which tests were clinically significant and which weren’t. The model couldn’t surface risk because it didn’t recognize what it was seeing.

These stories all end the same way: the AI didn’t break. The data did.
